Camper Forms

Thank you for your interest in participating with Camp One Step. Below are important forms to complete before your camper(s) attends a program.  Some of the forms are part of the electronic application process, but they are also available here to review.  The forms include:

  • Camper Medical Consent – Medical Consent forms are a mandatory portion of camper applications for all programs besides Family Camp and Brain Tumor Family Camp.  They need to be completed by the camper parent or guardian.  The Medical Consent form is part of the online application so it is not required to complete this version, but it is located here for review.
  • Camper Physical – Campers need to have an active physical turned in (completed by physician) to Camp One Step to participate in programs, not including Family Camp or Brain Tumor Family Camp.  For campers on therapy, a new physical needs to be submitted for each program.  For campers off therapy, physicals are active for one year from the appointment date.
  • Sibling Camp Medical Consent – Medical Consent forms are a mandatory portion of camper applications for all programs besides Family Camp and Brain Tumor Family Camp.  They need to be completed by the camper parent or guardian.  The Medical Consent form is part of the online application so it is not required to complete this version, but it is located here for review.
  • Sibling Camp Physical – Sibling Campers need to have an active physical turned in (completed by physician) to Camp One Step to participate in Sibling Camp.  Physicals are active for one year from the appointment date.
  • Camper Guidelines – All campers who attend a program are agreeing to follow the guidelines listed in the Camper Guidelines  The online application requires a signature agreeing to these guidelines, so the handbook is also located here for review.
  • Conference Point Center (CPC) Waiver – The CPC form is required for all programs that take place on the Conference Point Center campus.  Those programs include Sibling Camp, Summer Camp, Family Camp, and Winter Camp.  The CPC form is part of the online application process, but it is also located here for review.
  • Flashes of Hope (FOH) Waiver – The FOH form is solely for campers participating in Summer Camp.  The form allows a professional organization to come on campus and take studio-quality photos of campers to be sent to families after the completion of the program.  It is not required to complete the FOH form; it is an optional opportunity in which campers can participate.

Safety is our number one priority, so all of the above forms are crucial to ensuring that campers are enjoying their time away from home.  We appreciate your understanding that our goal is to provide the safest experience possible.
